Are these spawned based off of a number of mobs killed or is there a trap type like exploring triggers you have to pass to get then to trigger/spawn?

Probably about right. If you clear every mob from zone in till it spawns you generally get to the first flight of stairs past Harrier. Usually spawns about right there everytime.

they can be 1grped now pretty much with Demiplane T1 gear or so.

Yeah we one group it usually 3-4 days a week, not sure if any of you have tried it but the XP in CoA with a single group is mind blowing.

One run I got almost 3 LAA's.
